Elderwood at Hornell is seeking a dedicated, compassionate and collaborative Adult Daycare Manager to join our team.

Adult Daycare Manager - Position Overview:

    The Adult Medical Day Services Manager is responsible for providing social services, organizing services for registrants, oversight of day-to-day management, administration of services, and assigning adequate and appropriate licensed personnel to be on duty at all times.

The skilled nursing facility, just 55 miles south of Rochester, NY, offers an experienced and dedicated staff that cares for residents who need long-term nursing home and memory care. Comprehensive physical, occupational and speech therapy are provided in an updated therapy suite. Elderwood at Hornell also provides Adult Medical Day Services to members of the community.

Responsibilities

Adult Daycare Manager

1. Ensures that psycho-social needs are assessed, evaluated and recorded, and that services are provided to meet the identified needs as part of the coordinated care plan.

2. Ensures that staff members arrange for the use of and/or access to other community resources as needed and coordinate the needs of the registrants with services provided by the adult day care program and other health care providers, community social agencies and other resources.

3. Arranges for or provides services appropriate to each registrant in accordance with the interdesciplinary needs assessment and comprehensive care plan.

4. Ensures the following program components are available:

* Case management, including health education;

* Interdisciplinary care planning;

* Nursing services;

* Nutrition;

* Social services;

* Assistance with activities of daily living;

* Planned therapeutic or recreational activities and,

* Pharmaceutical services.

5. Provides social services ensuring registrant psycho-social needs are assessed, evaluated, and recorded, and that services are provided to meet the identified needs as part of the coordinated care plan.

6. Ensures arrangements are made for the use of and/or access to other community resources as needed.

7. Coordinates the needs of the registrants with services provided by the adult day health care providers, community social agencies and other resources.

8. Ensures that individuals registered in the adult day health program

* Have been referred by their primary practitioner and personally interviewed by program personnel.

* Can be adequately and appropriately be cared for both physically and emotionally and,

* Are not suffering from a communicable disease that constitutes a danger to other registrants or staff.

9. Ensures a comprehensive assessment is completed upon admission and every six months thereafter per regulation.

10. Regularly confers with Director of Nursing and Administrator to ensure appropriate oversight of the following services are provided in the adult day healthcare program:

* Medical services

* Nursing services

* Food and Nutrition services

* Rehabilitation services

* Activities

* Dental services

* Religious services and;

* Pharmaceutical services

11. Ensures a clinical record for each registrant is kept confidential, intact, and readily available.

12. Schedules adequately and appropriately licensed personnel to be on duty when the program is in operation.

13. Supervise direct care staff ensuring registrants receive assistance in accordance with their plan of care.

14. Provides or arrange in service education.

15. Develop Monthly Activity Calendar to address social leisure and psycho- social needs of the patients. This calendar can be an alternate version of the facility's calendar.

16. Monthly Activity or Program Calendar must be submitted to the administrator by the 15th of the month.

17. Manage and purchase program materials as well as ensure programs are prepared before scheduled times.

18. Obtain photography waivers upon admission.

19. Obtain monthly photographs of the patients and posted to Facebook or marketing materials.

20. Manage staff to address ADLs and programming needs and documentation throughout the day.

21. Manages and completes documentation of daily programs, nursing clinical needs, appointments, and ADLs daily.

22. Monthly audits performed on documentation and assessments. These audits are submitted to the administrator.

23. Weekly and Monthly reports are submitted to the administrator based on the following information:

a. Nursing Clinical Needs

b. Social leisure Program participation

c. ADLs

d. Medical Appointments

e. Scheduling of upcoming appointments

f. Assessment and documentation completion

24. Arrives to work on time, regularly, and works as scheduled.

25. Recognizes and follows the dress code of the facility including wearing name tag at all times.

26. Follows policy and procedure regarding all electronic devices, computers, tablets, etc.

27. Supports and abides by Elderwood's Mission, Vision, and Values.

28. Abides by Elderwood's businesses code of conduct, compliance and HIPAA policies.

29. Performs other duties as assigned by supervisor, management staff or Administrator.

Qualifications

Adult Daycare Manager

  • This position requires regular interaction with residents, coworkers, visitors, and/or supervisors. In order to ensure a safe work environment for residents, coworkers, visitors, and/or supervisors of the Company, and to permit unfettered communication between the employee and those residents, coworkers, visitors, and supervisors, this position requires that the employee be able to read, write, speak, and understand the English language at an intermediate or more advanced level.
